I'm going to broaden this to include all foam and not just CAFS, because I think that's what you meant.
I'm sorry to say, I think the use of Foam is sometimes decided by budgets. Some departments use foam on everything, others can't afford to.
In Medford we don't have any trucks with integrated foam systems. In my West Virginia FD 3 of our 4 engines have an injection-style foam system. We only use it with certain vehicle fires and flammable liquids.

In Tabernacle We have CAFS on the Quint. On every fire that I have seen it used or used it personally it has work very well. On a working fire the day after Christmas we had a 2 1/2 in service for about 5 minutes with the bulk of the fire knocked down before we ran out of water (tanker malfunction). For the amount of fire that there was it was very impressive to see how much fire was knocked down with that line in just a Short period of time. Even after we lost water the CAFS provided protection to the uninvolved area of the home that was covered with the solution and prevented the fire from spreading. From what I have seen I think CAFS works alot faster and better than just plain water.
