What is your company policy? One Tag? Two Tag? Three Tag? Or No Policy at all.
The current policy in Burlington County is it good enough?

2 tags one is left on the rig and then collected and brought to the IC. The other stays on the person and is dropped at the point of entry into the building.

3 tags. one in turnout coat for id purposes just in case, one left on truck, one at door

Ours is the same as Kramer411. Truck, door, interior coat pocket.

we have 2 one collected on the truck that goes to ic one for the entry point collector, i like the idea of a 3rd for id just hope it never needs to be used

3 Tag system. One inside the turn out for ID, one on the unit you ride on, and one to the accountability officer who's supposed to be at the door.

3 tags.....1 goes to the officer of your truck, 1 for entry into "hot zone", 1 kept in your right leg pocket.....info on tag.....name/birthdate/station, optional any medical info that the person wants on there ie. alergies to meds....

Ok well here's a question to add on here.
Does your company/department have a elected/appointed Accountability Officer?
Or is it the last guy who didnt pack out in time or something along those lines?

It seems everyone stays with the 3 tag system nowdays.
Just a thought...
If you have the third tag with medical information that's supposed to make it to the hospital with you, put this tag in your pants pocket or zip tie it to your suspenders. If it's in your coat pockets, it may not make it off the scene as you usually take your coat off first but almost always keep your pants on. Again, just an idea.

We've been using two tags the same way everyone else: one for the truck ring, the other at point of entry. This second tag's use is further defined by always being placed to the RIGHT of the entry point, be it a door, a ladder, a window, or a basement if an Accountabilty Officer has not been appointed or in place yet.
Having read all the above, I'm now revisiting the third tag idea...
Lastly, the Accountability Officer is usually a Junior or Probationary member, situated away from the IDLH entrance but within line of sight.

To expand or modify on what Sean said, our department used to sew them into the inside pocket with a few loops of nomex thread. I agree about the pants though, and it got me to thinking. Maybe we could attach them inside the waist of the pants or on the inside of the leg pocket with the aforementioned nomex thread. I would definitely not put that tag in a leg pocket without a way to secure it. Gloves and stuff go in and out of those pockets way too much to make sure it doesn't fall out. On a side note, one of the things I failed to mention about our inside pocket tag is that it has our picture on it so that if we are unconscious, someone can ensure that that medical info is matching up to the right person.

we actually had 3 tags and a information tearopen ID in the inside coat pocket.
