First thing is scene safety. Start PSE&G or AC Electric to the scene and try to get a rush on it. How many vehicles are involved? This one appears to be very serious in nature but are there other vehicles and patients that can be tended to while waiting for the electric company? How many total victims? Are any trapped or all walking wounded? What resources do we need? How many BLS, ALS, Rescues? Do we have enough stabilzation once we can access the vehicle?
If this were just a one vehicle MVC with entrapment: Assign command, ops, and a safety officer right away. Have power cut off. Remove or secure the pole. Stabilize the vehicle. Remove the bush. Patient placement in the vehicle is going to play a big part on the extrication. Remove the roof and windshield to get to patients in the front seats. If patients are in the back seats, cut the pole that is in the ground down so you can work around it. Then remove the rest of the roof or see if you can access patients through the rear of the vehicle. Keep an eye on stabilization throughout the entire incident.
